I bet this happened in Russia.
But having said that, I remember there was a rash of tires separating from their trucks about 10 years ago in Ontario. And it almost always due to improperly torqued wheel lug nuts... either under or way over.
And in at least one case, the tire went though someones windshield and killed the driver.

They had a problem with bus tires falling off when I was in college, leading to a local news story: The wheels on the bus are fall-ing off.
Those little neon indicators were quickly deployed across the fleet:
